DHOAS
IMPORTANT: You must attach your service records to your application for a subsidy certificate and get your Commanding Officer or relevant delegate to sign your application.
Please note that this application form can be saved elsewhere and completed electronically. You can do this by hitting the SAVE button, which is on the bottom left-hand corner of page 7 of the form. Alternatively, you can print out the form and complete the hard copy.
You can also fill out the DHOAS Change in Circumstances form electronically. The SAVE button is on the right-hand side of page 5.
Important information about Application for a Subsidy Certificate
The information you supply on this form will enable DVA to assess your eligibility and entitlement for the scheme. if you are eligible for DHOAS, you will be issued a subsidy certificate to take to your preferred home loan provider to establish your DHOAS loan. You must also submit a Subsidy Authorisation Request Form (SARF) once your subsidy certificate has been approved. See below.
To assist correct lodgment of your application, we have included a checklist at the end of the application form. make sure to attach all necessary supporting documents.

Important information about Subsidy Authorisation Request Form
Before DVA can authorise subsidy payments into your home loan account, you must submit a Subsidy Authorisation Request Form (SARF) to DVA. This is an additional security requirement and no funds will be provided to you until the SARF haas been lodged and your circumstances meet the requirements. It is in your interests to supply the SARF without delay, ideally as soon as possible after settlement and when your occupancy date has been determined. Please note that the SARF is not a substitute for the Application for a Subsidy Certificate form. It should be returned to DVA and not your your home loan provider.

Important information about Change in Circumstances
The strict conditions governing DHOAS mean that a change in your circumstances could reduce or end your entitlement to DHOAS subsidy. Relevant information is available at the following link: Change in Circumstances
It is your responsibility to familiarise yourself with the requirements of DHOAS and to advise us of any notifiable changes as soon as possible. This will allow us to minimise any negative impact on yiour subsidy payments. If in doubt, call a service adviser on 1300 434 627 to check.

DHOS closed to new applications with effect from 30 June 2010. Some former members who may have been eligible under DHOS may now be eligible under DHOAS.
