IMPORTANT: You must attach your service records to your application for a Subsidy Certificate and get your Commanding Officer or relevant delegate to sign and stamp your application.
Please note this application form can be saved on to your own system and filled out electronically. You can do this by hitting the "SAVE" button, which is on the bottom left-hand corner of page 7 of the form. Alternatively, you can print out the form and complete the hard copy.
You can also fill out the DHOAS Change of Circumstances form electronically. The "SAVE" button is on the right-hand side of page 5.
