Application for a Subsidy Certificate
To apply for DHOAS please complete the DHOAS Application for Subsidy Certificate form below.
IMPORTANT: You must attach your service records and get your Commanding Officer or relevant delegate to sign and stamp your application.
Please note this application form can be saved on to your own system and filled out electronically. You can do this by hitting the "SAVE" button, which is on the bottom left-hand corner of page 7 of the form. Alternatively, you can print out the form and complete the hard copy.
