What information will I need to complete the form?
To fill out the application form, you will need the following:
- Your contact details
- Full service details (attach service record)
- Your Discharge Certificate, if you're no longer in service
- A DVA Decision Letter, if you've been medically discharged for a compensable condition
You have to submit your service records with the DHOAS application form.
Your application will need to be certified by either your Commanding Officer, or an authorised delegate of the Commanding Officer.
The delegate is not to be below the rank of Warrant Officer Class 2, or equivalent. Applications will still be required to have the Unit stamp.
