What information will I need to complete the form?
To fill out the application form, you will need the following:
- Your contact details
- Full service details (attach service record)
- Your Discharge Certificate, if you are no longer in service
- A DVA decision letter: advising you that DVA accepts liability for one or more of the conditions for which you were discharged. In addition, you will need to submit a copy of your DM042 (Invalidity Retirement from the Defence Force Medical Information).
Your application will need to be certified by either your Commanding Officer, or an authorised delegate of the Commanding Officer.
The delegate is not to be below the rank of Warrant Officer Class 2, or equivalent.
