The Application for Subsidy Certificate can be submitted electronically via the online form or by printing the PDF version, completing manually and either sending by mail or scanning and submitting via email. IMPORTANT: Please ensure you attach your service records to your Application for Subsidy Certificate. This can be done electronically within the online form or by attaching a copy to PDF version. If the evidence of your service is not attached, the processing of your Application for Subsidy Certificate may be delayed.
DHOAS Subsidy Authorisation Request Form (SARF)
Before DVA can authorise subsidy payments into your DHOAS Home Loan Account, you must submit a Subsidy Authorisation Request Form (SARF) to DVA. This is an additional security requirement and no funds can be provided to you until the SARF has been lodged and your circumstance meet the requirements.
Please note that the SARF is not a substitute for the Application for Subsidy Certificate form.
Please complete the Subsidy Authorisation Request online form or print the PDF and submit to DVA, not your Home Loan Provider. If you do not receive an email receipt notification when you submit this form, please phone our Customer Service Team on 1300 434 627.
A change in your circumstances could have significant impact on your entitlement to DHOAS subsidy. It is important to familiarise yourself with the requirements of DHOAS and to advise DVA of any notifiable changes as soon as possible. This will allow DVA to minimise any negative impacts on your subsidy payments. Further information is available at Advise DHOAS of changes. The Change of Circumstances Form can be completed online and submitted electronically. If you wish to print off the PDF version and either send to DVA by mail or scanning and submitting via email, please do so.
